Our account shows as suspended

I am trying to login to our account and it shows as suspended.  

You were suspended because you have fields that are asking for credit card information. This is against our terms of use. I have removed the guilty fields and activated your account. 

Please avoid asking for credit card information and login credentials. If you want to receive payment or something similar, use our payment tools.
