If your forms are embedded to web pages, each time the page is loaded, the form will be also.  Each time the form is loaded, it counts as a view.  The form that is receiving the bulk of the views is the your contact form:

Also, if you are using any sort of website builder that has a live preview like Adobe Dreamweaver, each time the preview refreshes, the form will be reloaded and it will count as a view.

I reset your form views for the month to give you a chance to get this sorted.  Keep in mind, should you go over the limits once again, you would need to upgrade or wait until the monthly reset on the first day of the next month in order to restore your forms.
