Please share to us the URL of the form that have the issue.
I checked your account/username SMUEventServices but I did not find an existing form under the account.
I do see that it is a sub account to another account which have the forms.
Tell us the URL of the form so that we can checked the form for issue.
We will wait for your response.

Your autoresponder settings look good. It seems that the autoresponder has been recently updated, and I hope that the issue is fixed.
According to the autoresponder settings, the form user should receive two PDF attachments, one submission PDF file and the second custom PDF attachment file.
If the next user will not get those PDFs, please let us know. Thank you.
