Simple Form - Stopped Working - I am clueless - Removed Private

    I created this simple but effective form 4-5 years ago and have not touched it since.  It suddenly quit in July of this year (2016), was notified about a week ago.

    I tested your form and I'm getting this error:

    However, upon checking your website, I see that the way you embedded the form in your website is quite different and not using the default embed codes that we have? Could you please try removing the embed code on your website and get a fresh copy of the embed code under the "Publish" wizard, you can try using the default embed or the iFrame embed code. Here's a quick step by step:

    1. First, click the "Publish" button in your form builder.

    2. Navigate to the "Embed" tab, here you can see the different embed options that you can use. Try the iFrame embed code and paste it in your website's source code.
