Form embedded on Wix is not responsive on iPhone

Sorry again, but i have problem to fix my form in a mobil (i am testing in iphone) web. I have checked the responsive form in the setup form buy when i try to complete the form in my celular, its move all over, and some time it is not posible check the options and also de send bottom,

So I apreccied a lot your help again..

I have checked your web page on iPhone emulator and see the form is not responsive. However, when I check the form using the direct link, the form is displaying correctly and responsive.

The issue seems to be happening only after embedding the form on the wix webpage. Please try embedding the form using the direct link instead of iframe embed code.
