Yes how do I import that form into my form builder? 

Based on the other thread where you were provided with a demo form, you only need to follow this guide in order to clone the form into your account: How-to-Clone-an-Existing-Form-from-a-URL

It will clone the form exactly as it is seen when you open the given link, if the form has conditions then they will be cloned as well.
