Some fields don't work

Dear Sirs, 

I am using your JotForm for my accommodation bulletin for my rental, but for some reason 2 very essential fields don't work.  (Field 12 - type of travel identification document and Field 14 -country of issue (document)). You can fill the form, field 12 is shown as not filled up, field 14 doesn't appear at all on a filled list, like it doesn't exist.

What do you mean by "field 12 is shown as not filled up"? The field 12 which is label as "Identification Document Number and Country of Issue" can be filled without an issue:

And field 14 which is the submit button, will show only if the previous question is equal to "No"
