I would like to create a form with Math questions for students.

I need to add graphics with Math questions, and the ability for students to enter their answers.

Please advise.

We do have widget called "Math Graphs". But it is only drawing Mathematical graphics based on your configurations. It is unfortunately not a responsive widget which is able to allow your students to give answers on it. 

However you can alternatively use another widget. Draw on Image widget allows you to upload an image from web. Then users of your form can make drawings on this particular image.

You can upload an X-Y axis or whatever image you desire and your students can easily draw their graphs on the axis.
