Jot form does not allow me to create new fields in a form that has salesforce integration.
Previously, I could add new fields and match them to existing fields in a salesforce lead. (See attached screen capture)
But I can no longer do this. Is this a technical error? Could this be related to the new 5 form maximum on new accounts?

Try completing the "Other Information" field or removing it first. After doing so, the option to add a new field should appear once again.
