How do I use and existing PDF form?

    I currently have a PDF application form on my website.  I would like to be able to have applicants fill out the form, sign it, send deposit and submit, all on line.  How do I do this with JotForm.

    The application form I am using is :

    I truly apologize for my mistake on this.

    You can actually upload to JotForm your fillable PDF file and then use it within JotForm.

    Make sure you are also logged in to your JotForm at https://www.jotform.com/login before uploading to be sure it will be uploaded on your My Forms list.
