I have 2 addresses for my form and the information will not delete once submitted.

If I understand correctly, the Street Address and Street Address Line 2 is not being deleted once the form is submitted? I cloned your form and noticed that you have enabled the Autofill feature on your form. It could be possible that the data on the address was still stored on your browser, which is why they are still visible on the form. You can disable the Autofill setting if you do not want the data to be saved on your browser.

- On a different note, I noticed that you have credit card fields on your form. Please note that collecting credit card information using plain text fields is strictly prohibited in jotform. It is considered a violation of our terms, and it could lead to your account being autosuspended by our phishing detector.
To collect credit card information without charging them, you will have to use one of our available payment integrations such as Stripe or Paypal. Here's our guide on How to Enable Payment Authorization
