Form is disabled

    My forms are disabled, did I do something incorrectly?

    I checked your form and you were asking for username. Please avoid asking for login credentials such as username or password, it is against our term of use

    I've activated your account, but you might get suspended again. I suggest to remove or change the username field.

    how can I have them set a username then? I'm trying to use jot form for the functionality over my default forms.

    You can keep the "Username" field on that form. However, it is possible that your account will get suspended again by our phishing tool if you created another form that has a username field on it. If that happens, you need to contact us again so that we can re-activate your account.
