Take a look into our New Form Templates Gallery, you will find several pre-designed forms that you can use and customize to your needs.

Browse by form types or type the word "survey" and you will see 62 survey templates ready to be customized

You will mainly need to know how to customize the email notification, so please check this guide first, to see more details about how to customize an email template .

But, if you don't want to use our Form Templates Gallery, you can start it from the scratch.

For example, you can add some fields from Quick Tools Menu to gather names , emails , address, phone , etc.

Then you can go to the Survey Tools. And select what you need , see this guide in order to browse the available tools in jotform .

Feel free to look for further assistance, if you need it.

Ok, my fault , I didn't pay attention to your other question.

Once you finish the form , embed it into your page, additionally in order to see the results, you can create a grid report and embed it  into another section of your page as well

    Best regards,
