Email notification field did not show data.

    the following form notification did not show certain field.


    the notification email show:


    however in the submission data there was indeed data being entered in "contact hours..

    Hello theies,

    I did check your form's notification email alert and did not find any issue with that. It seems you have already solved this problem. I did test your form by sending a test submission and the submission email which was sent does display the answers correctly. Please check the screenshot below:

    I believe that we did not embedded. We however do have a link to the jotform.

    I did a test submission on the site ( you provided and I was able to see the issue you are reporting. In the notification email sent, the value for the "Contact Hours/ Proposed PDU (exclude break time)" displayed is empty. 


    However, in the submission data, the value for the field is 1.


    As per checking your site, it seems that you have embedded/publish your form to your site ( using the Form Source Code. Can you try to re-embed/re-publish your form on the site by using the iFrame code of the form and see if this will help fix the issue. To get the iFrame code of your form, please follow the instructions provided in this article: Getting the form iFrame code.
