HOW CAN I LINK A FORM WITHIN AN SPECIFIC LINK

I just wanted to know how can i link a form that i just have created to an specific link of our website, as for example: www.website.com/reporte

The easiest way to do that is to embed the form on a certain URL that you have set in your web page. Example, you have created a blank page www.website.com/reports, simply edit the contents of /reports page and embed the form there.
