I'm reading the document you sent us, but there seems to be multiple matters in the same document so assisting you on this thread would generate confusion and we would start trying to resolve multiple problems in the same thread. 

In your PDF doc I can see you have listed 5 questions, what I would suggest you is to open a thread for each one so we can properly assist you on each one and help you to resolve the problems you're having with your forms.
