Form not mobile responsive - please help!

    I can't get this form to become mobile responsive to save my life.

     

    Please help! Or please provide the proper code for it to work on a shopify theme created by Pixel Union.

    You have 2 options to choose from to make your form mobile responsive:

    1. Either follow this guide: How-to-make-forms-mobile-responsive 

    OR

    2. Add the Mobile Responsive widget to your form
