How can I add a unique title to the Terms and Conditions widget submission?

    What we're looking to do is either:
    a) Create a title field that shows up in the email submission (right now, it doesn't show at all; just the word 'accepted' in the second column). If we attempt to add a title manually, it won't show.
     or
    b) Change the phrase 'accepted' to something else (such as 'EULA accepted')--it's too vague for our purposes in order to verify a registrant selected the terms and conditions box.

    What can we do?

    How about using another widget to construct the text/title you want. It will do appear on another field/column on the resulting submission -- but I do not see any downside to that.

    Example, use the Calculation widget to have another field become the container for the title/text of the Terms/Conditions widget.

    You can just hide the calculation widget so that it will not be seen on the live form.

    I believe this workaround addresses the 2 options (a,b) you mentioned.
