Do you have a club memberhsip form that collects credit card details?

HI I AM TRYING TO SET UP A MEMBERSHIP FORM THAT OUR WINE CLUB MEMBERS CAN FILL IN AUTOMATICALLY AND THEY DO NOT HAVE TO DOWNLOAD THE FORM, SIGN IT AND AND THEN SEND BACK TO ME.

I AHVE ATTACHED THE PROFORMA I WAS PLANNING TO USE, THOUGH I CAN GET AROUND SIGNATURES IF THEY JUST TICK BOXES.  BUT I ALSO NEED THEIR CREDIT CARD DATA FOR BILLING - IS THER A FORM THAT DOES THIS TOO?

Thank you for your interest in using our product. JotForm is an online form builder, it allows you to create online forms that you can use for orders, payments, registrations, etc...

However, please note that we do not allow forms asking for sensitive information like credit card details using plain form fields (ex. text boxes). If you wish to ask payment from your users, it is a must to use our payment integrations (ex. Paypal, Stripe, Authorize.net, etc...). JotForm does not process these payments, hence we do not store the sensitive information like credit card info in our servers.
