As i've check you form and made some test, I don't find any problem. You can see it in your submission page. Regarding on that issue, can you please try changing your Sender Email of the notification to firstname.lastname@example.org
To add email@example.com to your sender email
1. Edit your form
2. Click Setup and Share tab
3. Click email alerts
4. Click Notification
5. Click the Reply-to and Receipt Setting button
6. An enveloped form will appear.
