    I believe the issue is that your account is a Guest account.  The subscription and log in details should not have been allowed to apply to your account.  Since the account is a Guest account, the purchased theme or any theme for that matter, cannot be applied:

    It is also not possible to apply the theme via the theme store since the full list of your forms does not show:

    I will forward this to our developers to have them take a look.  We will let you know via this thread as soon as we have any updates.

    Why is this a guest account. I paid my subscription to Jotform.

    Your subscription is properly applied to your account and there are no issues with that or your forms.  Your account was a Guest account that was not fully registered before the subscription was applied.  Everything I checked looks to be the same as if you would have registered the account from the start with the exception of applying themes. 

    We will let you know as soon as we have any information regarding the status of the theme issue.
