Radio Buttons are not shown when my form is embedded

    Also please tell me how I can get this form to show completely by scrolling on the webpage, NOT scrolling on the form within the webpage.

    Can you please give me the exact location where your form is embedded on this website:

    You should still try to re-embed your form using the iFrame tough. It should resolve your issue. 

    The other issue you are having has been moved to another thread. When I can reach the exact location of your embedded form I can provide further assistance on this issue too.
