Why is my form disabled no matter what I do?

    Seems like no matter what I do, clone, disable and re-enable, no matter what, the form I have just won't be "Live" even in testing the whole thing. It keeps saying it's disabled. HELP!

    Hello,

    I've checked your account and also your form. I've seen that your account is auto-suspended due to phishing.

    I recommend you to read our terms of use. You can not demand password in your forms according to our terms of use.

    I've deleted forbidden field from your form and I've re-activated your account.

    You can ask them to designate the password through email. We just can't allow forms that would collect this type of information from someone. Or, why not enable membership in your Sharepoint website? If that's for your website then I'm sure you can set it up there. You can ask the basic information using Jotform, do the password thing on your website.
