I have copied the code to place the form in Dreamweaver (CS5) and everything except the checkbox appears as shown in the preview of the code in JotForm's design tool. Can you help me figure out why that one element is not working as it should be? Thank you.

Hey @sgalhc, I checked both the page where you embedded the form and your standalone form but failed to reproduce the same issue you're describing. Although I'm seeing on the screenshot you attached the the checkbox is indeed missing, it wasn't the case when I checked it. It was visible on both forms (standalone and embedded).
Here's how it looked like from my perspective:

Is your issue resolved? If not, I strongly suggest you reload/refresh your website and try again. If that won't suffice, and although this isn't necessary, only clear your browser's cache and cookies if reloading/refreshing the page won't help.
