Why are All my jotforms coming up as disabled?

Hello,

I've checked your account and your forms. I've seen that your account is auto-suspended due to phishing as you wanted credit card information from users in your form named "Majestic Dream Vacations" without using our Payment fields. You are not allowed to do that according to our terms of use.

I've deleted that forbidden parts from your form. Then, I've re-activated your account again.

Also, I need to ask you that why do you need "Credit Card Number Last 4" of users in your form named "Payment Authorization Form"?
