I have checked your form, and I am able to reproduce the issue.
Seems there is some problem in the Infusionsoft integration.
Can you please try to re-integrate your form's integration to Infusionsoft (remove the integration and integrate again) and see if it will fix the issue? You might also need to generate a new API key for your integration: http://ug.infusionsoft.com/article/AA-00442/0/How-do-I-enable-the-Infusionsoft-API-and-generate-an-API-Key.html
