Form is displaying an error - currently unavailable

I created a Model Release form pdf to add to my website.  I followed the instructions to add the form to my website and it is not working.  Even just to "view form" in jot form it say gives me an error message saying it is not working.  Can you please help me get this up and running again.

I have checked your account and see the account is active and in good standing. I have checked the other forms and they seem to be opening normally. However, the form in question Model release - website is disabled in your account and so it is displaying an error.
