Why changes to my form are being reverted?

I have a form that I have fixed several times. I appears to be removing a drop down "Form Type" and Conditional logic as well as changing my heading. It's as if it's defaulting to old versions on the form. What's going on here?

The form is title Media Request Form.

I have checked your form and I can see the heading text is there, but the font color seems to be the same like the background, it is currently white: 

What I would suggest you is to check the text color as well as the injected CSS code in order to verify that nothing is affecting the header:

Also, I was unable to find the field you're mentioning "Form Type" and there are only two conditions in your form, they seem to have a missing field something that I would suggest you to correct in order to avoid the form to work inappropriately.
