I used a template and made changes but having issues

    The questionnaire form I used in templates was successfully personalized with my photo and questions. However, the submission form sent back to me has the original templates photo on it instead on mine.  I cannot figure how to change it on jot form so my picture shows up. 

    I did not upload to a website.  I inserted the link in an email.  I tested it on myself.  It worked perfectly until I received the submission with the incorrect pic.

    The old image shows in the email alerts because the email notification wasn't modified along with the form.

    There are two ways you can update / modify the email notification on your form.

    Method 1: The easiest way is to remove the old email notification, and then set up a new one in its place:

    Method 2: Another way to fix the issue is to simply update the image inside the old email notification. To do this, click on Emails, and then on the Edit button of your email notification:

    Once it loads, you can click on the old image, and use the Insert/Edit image button to change the link to the image that is currently being used:
