I was having issues that every time i would hit the link my forms would be disabled. The last message you guys gave me was my forms were fine and don't know why they were disabled and the next form was fine. But before putting on my site I thought I would try a new form and again they are disabled.

I have checked your forms and reliazed this form contains Password field.
You can not directly get personal sensitive information such as password, credit card details. If you insist on collecting any of these information, your account will be suspended again.
I have deleted this field and reactived your account.
