The visual report is not pulling in answers from the survey form I created. I believe there is an issue with your software.

With which form you are having this issue with?
I did check your last edited form "icomplete.com Client Feedback Form" and found that you have added a visual report but this form does not have any submission data.
The other form you have "Survey Form for Active Cleaning - do not delete" was last edited on Sep 29. Is this the form you are having?
The reason why you are not able to see the submissions in report or in your form's submission page as well is because the owner of form "icomplete" is saving the data in "Europe Datacenter". I would suggest you to please enable the "Europe Datacenter" in your account data page and then sign in to your JotForm account using following URL: https://eu.jotform.com
