Calendar popup not working correctly?

Hi,

Our form (https://form.jotformeu.com/62726952253359) is misbehaving. I have removed some of the elements which did not show correctly (it used to have conditions set, which worked earlier today, but I had to remove becuase options did not show/hide with a given state). Anyway, now the problem is that the calendar does not pop up. Is there a bug in our form, or is there some error on your side?

I just tested your form, and it seems like a heading field was corrupted, which is the reason why the calendars do not pop up, please remove the following heading field, and add a new one:
