How to create a Electronic Payment form like that one attached?

I have tired to send this questions and keep getting Spam error message. for the past two days.

I would like to create the same form that is attached. This form was shared with me through another Girl Scouts Council and we would like to  create the same one with some minor changes.

I looked under your templates but did not see this template anywhere. can you please assist me with this.

You can actually clone the form through the URL. Here's a guide: How-to-Clone-an-Existing-Form-from-a-URL
