Electronic Payment Form

Hello My name is Sunny,

I am with Girl Scouts of Connecticut and I am trying to create a Electronic Payment form for our Troops.

the form with give us authorization to debit their bank account for their cookie sale. So we will need all of their banking information along with their personal information and troop's information.

I have attached a form that the Greater Atlanta Girl Scouts kindly shared with us and we would like to create the same form just with some minor changes.

Can you please assist us with where I might find this form on your site.
