How are we over the quote? This is the second time in the last 2 months....
Google Analytics only shows that we have 3k sessions this month....and the form is NOT on every page.
We need this up ASAP

I've checked your account and I've seen that your account is over limited as you have 11542 form views this month as you can see the screenshot below:
This is the reason you are out of form views. So that, you should upgrade your account.

You can check your form analytics to learn how many times your each form has been viewed as you can see the screen below:
As I've checked your forms:
Infographic Form has 19 views for this month.
Careers Form has 484 views for this month.
Sales Form Widget Eldercare has 676 views for this month.
Sales Rep Form has 12,664 views for this month.
Sales Form Widget has 2,468 views for this month.
Healthcare 2020 Discovery Workshop has 0 view for this month.
Product Landing Page has 99 views for this month.
If we calculate for all, you have 16,410 views for this month in total. So that, this is the reason you are out of form views.

some how we are at $1,000 views on the first day this doesnt make sense??? is our forms being spammed? We dont even have 100 website visitors in a day

Firstly, your account limits have been restarted on 1 November 2016 as our monthly limits restart on the beginning of each month. So that, your account is not over limited now.
Secondly, just in case I have forwarded this threat to our software developer team in order to check if there is an issue in our analytics. We are going to inform you via this threat if there is an error in the analytics.

yes we need your help..our Sales Rep Form accumulated over 500+ views in less than 12 hours....it doesnt make sense??? We don't get nearly that many sessions on our site per day??

Please share also to us the URL of your website where the form having issue was used.
It is possible as well the embedded form have script conflict issue that was causing the anomaly in the form view accumulation.
We will investigate further once we know more details.
Currently, the ticket report submitted by Candy had already been assigned to one of our developer. The developer assigned will provide response here as well once available.

I did some investigation into the problem and I don't think there is a problem on our end. However, I did find something you should check on your end.
On your website http://www.napierhealthcare.com/ you have a 'Talk To Us Now' button which opens your form in a pop-up modal.
I pressed the button once and it registered 6 views on our end for the form. Please check the code on your end as I believe pressing that button is loading the form multiple times which is causing many views to be registered.
Below I have included images of the button I clicked on your website and the multiple views registered on our end.
