How to attached fill-in boxes on logos in JotForm

    Within the current JotForm I have at www.tbsportsentertainment.com, can I insert two logos, one of LYFT and the other of UBER.

    When the user clicks on either logo, a tool-tip appears, where within the tool-tip (Or pop-out browser), the user will submit the to and from address, and submit, which I will get.

     

    Once I get the information, I can order from either platform, and have the actual driver sent to our members.

     

    Can that be done? If so, how would you recommend I enter the two company logo and have an attached fill-in boxes attached to each?

    Hello,

    In a nutshell, I want to create two separate jot forms, both with a to and from address. The job form will be connected to a small 40 x 40 logo of both Lyft and UBER.

    After the user fills in the address information, we will order the service on either of the ride-sharing platforms, and have them pick-up the rider.

     

    When a user clicks on one of the logos, a tool-tip (pop-out box) will appear where the user will fill-in the address information.

    Hi,

    I believe you can achieve your requirement by adding the logos to your webpage and connecting LightBox forms to them. Please see How to Create a Lightbox Form for more information.

    This way, when someone clicks on either of the logos in your webpage, a connected form will popup.
