Why has my account been suspended?

I am developing a website and my account was just suspended because it says I was phishing.  I assure you I was not phishing, I am simply setting up JotForms for parts of the site.

The Phishing detector have suspended your account and forms because you have form that ask for account/email username and password. This violates the Terms of Use.

I suggest you delete this form and all other similar forms on your accounts. We can request for reconsideration to re-activate your account after you have complied with the Terms.
