There are some field that do not work with the PDF version: https://www.jotform.com/help/312-Fillable-PDF-Everything-you-need-to-know-about-JotForm-Fillable-PDF-Creator

However, the image field should display properly when the form is converted to a fillable PDF form, please let us know what is the form you are working on, so we can check further.

    Hi there,
I only have 2 forms on my account. I can set them up and the check box
tables all look in alignment but when I submit the view is different and the
    boxes are not aligned.
Also how do I send a copy of the form to customers? Can I get my service
techs to fill it in on their tablets, submit it with the customers signature

What do you mean by "when I submit the view is different and the boxes are not aligned.", please share a screenshot to illustrate what you mean:

On regards of sending a copy of the form to the customer, I guess you are referring to sending a copy of the submission, for that purpose you can setup an autoresponder: https://www.jotform.com/help/26-Creating-a-Form-Autoresponder

For signatures, include the E-signature widget to your form: https://widgets.jotform.com/widget/signature

    Hi There,
below is a copy of my form filled in before I submit it. if you note the top
check box is wider than the other one s below. I have tried to change it but
I cant. I even deleted the whole top section and duplicated the next one
down but the check box columns ended up wider again.
Now here it is after submitting. My header does not appear and nor does my
Logo. The columns are not aligned. The font is all different.
How is this document stored on your server and what does it look like when I
    send it to my customers?
Do I save it as a pdf view and submit it or is in HTML file format?
Our technicians complete this form on their tablets in the field, when they
submit it we want it to come back to our head office so we can file them and
then re send the inspection form out with the customers invoice. How do I
    make that happen?
Do you have someone in Australia that can help me? I am willing to pay a
consultant to set this up for me as we need a paperless system urgently. Can
    you please help me?

    Hi again,
this is the other form I set up. I tried to set up a click box to show what
    items passed the inspection.
This is the form that is emailed to me after submitting. Again the fonts are
different, the check boxes have disappeared, and it is not in a format I can
    send to my customers.

I'm afraid we didn't receive your attachments, Neil, as our forum software cannot accept inline attachments sent through email. In order to include screenshots in your message, please open your support thread in a web browser, and then upload it by following the guide my colleague linked to earlier:

    https://www.jotform.com/answers/414264-How-to-include-screenshot-image-in-the-support-forum

Your current support thread is here:

    https://www.jotform.com/answers/969166

I have also cloned your form into my account for checking, and upon generating a PDF form out of the cloned form, there wasn't any difference in size between check boxes:

I also noticed you mentioned that the form you receive through email looks a bit different than the form looks when it is being filled out. This is normal, as the PDF file of a submission that you receive through email is generated through the PDF generation process, and cannot fully resemble the online form.

If you wish to print your submissions in a way they look exactly the same as when a Jotform form is being filled out, then the only way to achieve it is by following this guide:

    https://www.jotform.com/help/335-How-to-print-a-form-exactly-as-it-is-when-viewed-but-with-the-user-entered-data
