I've previously done this and it used to work fine. I've taken another look at the form and it's no longer accepting html.
On the last page please hover above 'Facilities and equipment' file upload option.
Please come back to me asap as this is disrupting our services.

It's now been over month with no reply at all. Not even an update. Can you please give me a progress update as this is dragging out too much now. We pay for our services with you and don't feel this is acceptable.
We are aware of the issue and there are other users that have reported it. Our developers will sure be looking into it but the timeframe is completely dependant on their current workload.

I am paying for this service and customers are continually complaining.
I have waited for over a month for this to be fixed, it is not just a case of simply going to a new provider, my forms are embedded within so many different websites. I am now looking to formally complain, I understand you have capacity issues. However, this is something that should have been tested before hand and other jot form customers are experiencing the same issue. This needs to be a priority!
Kindly send me your complain form as I am looking for compensation as a paying customer...
