Is there a way to export the information on separate pages of a survey to separate rows in a spreadsheet?

I created an ordering form for teachers who need art supplies to teach their classes.  Each page of the survey is a different item.  My goal is to upload the info from the forms so that I get each item's info (each page of the survey) on a separate row.  When I tried uploading the data, it put the entire form data on a single line.

    Example of each page:

What is the item you want to order:

    What class is it for:

    How many do you need:

    Price:

    Url of where to purchase:

Ideally, I would like to gather this data like this, where each ROW represents info gathered from a different page of the form:

    column 1               column 2                        column 3         column 4         column 5

    Pencils              drawing class                        6                    $5                 www.amazon.com

    Paper               drawing class                        1                    $10                 www.amazon.com

    Charcoal           drawing class                        2                    $3                 www.amazon.com

    Anyone have any ideas?

If I am understanding you correctly you mean that you want to reformat how your submission data is displayed and viewed in a spreadsheet.

It seems to me that you want to have it stacked only showing the relevant data in your spreadsheet?

The way it functions for excel currently is each column in your form is for your fields then each subsequent submission you receive will fill in your rows with the answers and there's not really a way to customize it unless you use a formula or copy and paste to change it.
