My embedded form stopped working on PC

Hi, 

All my forms are working except "Surrender" people on a mac it goes okay. On PC on several computers not working. Any suggestions/can you help? We have a few dogs that are incoming that we need the correct paperwork for and our rescue president is more a dog person than a computer person.
