Jotform not showing on Mobile Site

My Gift Voucher Jotform is only showing Buyer Name, Email and Contact and none of the other information?

It's showing all of it fine on the desktop but I can't work out why it won't work on mobile.

We've had a number of people call in the last couple of days who can't purchase online. Help!

I did check your webpage where you have embedded your form and found that you have used your form's script embed code. Please note that for your embedded form's to be mobile responsive, we recommend you to embed your form using its iFrame embed code.
