Keeping texts and unselectected options showing on final PDF forms.

I have a question about making forms . I already signed up for paid subscription under user name thulali. When I make a form that has a consent , and a free text , followed by name and a signature. Now when the customer filled it out and sign st the end , and we try to print it or make a PDF file out of it it appears within the text . So there will be a name a signature without the text , meaning without mentioning what the customer ( patient in our case ) is consenting for , so the from is useless in that case . Also when we ask the patient to pick what his child can do , and add choices , let say he picks 5-6 out of 10 , again when we try to Create the PDF it appears without the options he didn't choose . I am wonder if there is a way to keep the unchosen options listed but unchecked , and whether we can keep all free texts showing on PDF documents at the end . I am sure there is a way but I can't figure it out . Please let me know to whether I can go forward with creating forms or not 

Are you referring to the downloaded PDF from the submission page? If so, to show empty fields and text on the submission PDF you will need to disable the Auto Hide Empty Fields option and enable the Show Headers and Texts option in the submission page. Disabling the Auto Hide Empty Fields option will display all available fields in your form in the submission page and also in the submission PDF. Enabling the Show Headers and Texts option will display all headers/text in your submission page/PDF.

If you are referring to a different concern, can you provide us more details so we can further check the issue.

Are you referring to the submissions on this form (https://form.jotform.com/62961056614154)? I checked the submissions there and downloaded them as PDF file.

It appears that the empty fields are shown on the PDF file without any issue. Can you please send us the screenshots again or provide us with steps on how to replicate the issue on our end.

Unfortunately there is no universal setting to show empty fields or show the header texts on all your forms. You will have to manually set the options in the submissions page of each of your forms.
