The application only works part of the time. In the screen shot it show a field for SSN, not a question on the application? Source code shows errors?

Your notification template is customized, it's not an error. The email template will auto update itself with fields added in your form, but this feature won't work for customized layouts. If you don't have an SSN field, you are free to delete it from the email editor.
To delete, simply place your mouse cursor on the SSN column then click "Delete Column" option:
