Why my forms are disabled?

All of my forms say "Form not Found. This form is disabled".

Can you help me fix this?

I checked your account and it was autosuspended because of our anti-phishing system that detects suspicious forms and disables accounts. 

I checked the suspicious form and problem is in these fields:

It is against our Terms of use to create login forms meaning collecting usernames and passwords on a form.

I removed those fields so please check your form and refrain of putting them back or your account will be auto suspended again.
