Why are my forms disabled?

Hi forms was working a few hours ago.

now its says Form Disabled.

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I notice that your form MAC4U Workorder form is collecting username and passwords which is not allowed with JotForm. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

I have removed the guilty fields from the form and activated your account. Please refrain from adding such fields to your forms to avoid any further suspensions.
