Is it possible to connect my sequel pro database with jotForm ?

I am not sure if I have understood your question correctly. Do you mean to say that you want to send your form's submission data to your own MySQL database?

Unfortunately we do not have built in feature to send submission data to your own database but with a little workaround it is possible to achieve your requirement.
