My form submissions are no longer coming through to my email.

    I checked our logs for the addresses in your form and the notifications do not look to be trigger for new submissions.  The email are conditionally sent, however, the condition to send an email was met in your last couple submissions, they should have triggered emails. 

    Try creating a separate condition for each email address instead of a single condition for multiple addresses:

    I cloned your form to test it and your setup did look to work properly.  I am not sure what is causing the notifications not to trigger, with them separated, we can at least rule out multiple addresses in the condition being the cause.  Though multiple addresses are working from my testing.
