the form has information that I would like removed. How can I do this?

when i look at the form the information i don't want is not there initially.  when I imbed the form I see something at the top of the form, like the name of a company.  

Open your form in the advanced designer:

From the advanced designer, remove the form cover:

That will remove the image at the top of the form.
