Hello JotForm, I hope that your day is going well.
I was wondering if you can help me with an issue that I am facing. Recently, I had one of my web guys create my new website (welendyou.com) and integrate your web form. Normally, the leads were forwarded to me however they have stopped.
I tired entering all usernames: firstname.lastname@example.org, email@example.com, firstname.lastname@example.org, email@example.com, firstname.lastname@example.org, email@example.com to do a password reset and none of these have worked.
Could you let me know what the username so I can reset my password and see where the leads are going to?

PS - Is this how the form is suppose to be come in? Sometime it shows this while other times its more of a standard text email? Also who is firstname.lastname@example.org?? Is somebody getting CC'd on my incoming leadsPage URL:

I was not able to find any JotForm forms embedded to your webpage. I was able to find what I believe are your forms in the account "Tunuva01". This account is associated with the email address "email@example.com". You may need to contact your web guy for access to the account.
I am going to assume that firstname.lastname@example.org is an email address they set up for your account and the wly stands for We Lend You, your companies' name.
There are a few forms in that account that have been deleted, the most recent active forms look to be sent to "email@example.com".
